Skilled Nursing: Registered Nurse(RN)/
Licensed Practical Nurse (LPN)
A Reliable Care
Home health care nurses treat our patients in their homes. They may work with patients who have long-term conditions or disabilities, or with patients who only need care for short periods after an illness or accident. Their patients may be unable to leave their homes for treatment, or may have home care as an alternative to treatment in a hospital, clinic or physician's office. The primary type of care associated with this role is nursing, however home health care nurses also have other responsibilities as follows:

-
Routine monitoring tasks, such as taking blood samples for tests or temperature, blood pressure and pulse readings, or they can be more complex
-
Certified LPN/RN may dress wounds, administer medication, give therapy and monitor catheters and other medical aids. In some cases, they work on an ongoing basis with patients with critical or chronic conditions.
-
Our nurses may help people with dementia, cancer, cardiac problems and may manage home dialysis and ventilator treatments,
-
Home health care nurses also are responsible for assessing, monitoring and evaluating the patients they visit and for checking on how they are coping at home. Nurses often are part of a larger team, working with doctors and other care staff.
-
Our Nurses may train patients in how to use equipment and aids, and give advice on taking medications and managing side effects. As part of their assessment responsibilities, they may give advice on diet, exercise and ways to cope in the home environment. Nurses are required to record the care they give for each home visit according to the guidelines of our agency and state and federal regulations.
